In preparation. This notice is being finalized from the Board-adopted instrument and is not yet approved for publication. It is a longer and stronger set of promises than our website privacy policy, which covers only your visit to this site.
Ask us and we will send you the current notice before you begin care. You will also receive it in writing at intake, and you will be asked to acknowledge that you got it.
If you have a privacy concern now, you can raise it with us at any time under our complaint procedure, and you can take it to the U.S. Department of Health and Human Services, Office for Civil Rights. Nothing about your care changes because you complained.
